New Patients

You can now register to be a patient with Ash Trees Surgery on-line. Please click on the link below and start the process. You do not need proof of address, ID or an NHS number. Once you are registered you will receive an email from the practice.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

Online Registration Form

You can still register in the surgery via a paper form. You are required to fill out the two forms below. Please print and bring in to surgery once completed. Please note, anyone over the age of 16 will need to present a form of ID along with their forms. We will accept passports, driving licences or two forms of utility bill, with both name and address clearly stated. You can also collect a paper form from any of our surgeries.

Once we have received the fully completed form, either on-line or paper copy, your registration will be processed within 1 week.
